    1. Open the Smart Jira Forms Issue Checklist Template and Report add-on. Create a new form checklist or use an existing one.

    2. To set up automated form checklist addition access Add to issues automatically button and select the desired projects and issue types from the dropdown list in the modal window.

    3. Once the selected issue type is created in the necessary project, the form checklist will be added automatically. Now you can fill the checklist directly in the issue and submit it when it is done.
