Open the Smart Jira Forms add-on. Create a new form or use an existing one.
Go to the Settings tab and click the Destination Field setup. Press Add more fields, choose a few form elements (due to Destination Field Characteristics), and select for each of them the same issue field. Also, relate form elements to the needed Jira or Custom fields.
Save the form and spread it in a convenient way (Automatic Form Addition to a Jira issue, Manual Form Addition to a Jira issue, (JSM) Customer Portal Form Addition). In the next gif, it is shown what happens once the respondent fills out the form and submits it, form element values will automatically map to the selected Jira field in the existing issue.
