Build the form for each step using typical form elements: Text field, Text area, Action Item, Dropdown, Multichoice, Checkbox, Radio button, Date, Date and Time, Attachment, URL, Number.
Each form element has its setting. Add a label to the name field and add the help text to explain what information should be provided in this field.
Save the template after setting it up.
