Consent checkbox
A Consent checkbox is an essential element in forms, enabling users to explicitly agree to terms, conditions, privacy policies, or any other required statements before proceeding. This small checkbox, accompanied by a descriptive label, serves as a visible agreement mechanism that ensures compliance with data protection regulations such as GDPR, CCPA, and other legal requirements. By using a Consent checkbox, you create a clear, auditable record that users have read and agreed to the specified terms.
The primary purpose of the Consent checkbox is to secure explicit user consent, ensuring compliance with legal obligations before collecting or processing personal data. This protects both the user's rights and your organization from legal liabilities by documenting that consent has been properly obtained.
Instruction on how to add a Consent checkbox to the form
Open the Smart Forms for Jira add-on and either create a new form or use an existing one.
Add the Consent checkbox element to your form. Customize the accompanying text, which can be up to 255 characters, to clearly outline what the user is agreeing to. If needed, include a Related URL linking to detailed information, such as terms of service or privacy policy, to provide further context to the checkbox.
Once the form is distributed (via email, added to the Jira issue, embedded on a website, etc.), the assignee can review, fill out, and sign the form as required.
Upon submission, the form becomes non-editable, preserving the record of the user's agreement.
