Form & Content elements

To build the most useful form for filling out, utilize the Form and Content elements along with the following settings and descriptions to customize it according to your specific needs.

Each form and content element has a unique key, making it easier to work with other settings when form elements have identical labels.

Form elements

Single-line text input is usually used for capturing simple responses. It is suitable for inputting short text such as names, surnames, locations, and mobile phone numbers.

Multi-line text input is used when multiple lines of text or detailed explanations are required. It provides a larger space for users to enter comments, descriptions, or other lengthy responses.

Consent checkbox allows users to explicitly agree to terms, conditions, privacy policies, or other statements, ensuring compliance before proceeding. The label next to the checkbox clearly explains what users are agreeing to, providing transparency and clarity.

Dropdown allows for selecting a single option from a predefined list. It presents a list of options in a drop-down menu format. This element is commonly used for selecting options like company departments, cost centers, or cities from a predefined list.

Multi choice is used when users need to select multiple options from a predefined list. It presents a list of options in a drop-down Multiple choice menu format. This form element is useful when users need to make multiple selections from a list of choices, such as selecting multiple categories or tags for an item.

Checkboxes are used when users need to select multiple options from a predefined list. Each option is represented by a checkbox, and users can select or deselect the checkbox as needed. For example, when filling out a form for preferences or interests, users can check multiple checkboxes to indicate their choices. These checkboxes can be arranged either vertically or horizontally, offering flexibility in form design and enabling you to customize the layout to meet your specific requirements.

Radio buttons allow for selecting a single option from a predefined list. It presents the options as selectable buttons. This element is commonly used for selecting options like company departments, cost centers, or cities from a predefined list. Like checkboxes, radio buttons can be arranged vertically or horizontally, providing added flexibility to tailor the layout and enhance the overall user experience.

If the Radio Buttons element contains more than 5 options, they will be automatically arranged vertically.

Date provides a date picker to select a specific date. It is useful when users need to submit dates like release dates or hire dates. Users can easily navigate through the calendar and choose the desired date.

Date and Time provides a date and time picker to select a specific date and time. There is a time template with 30-minute intervals but any needed time can be entered manually. It is used when the submission of both date and time is required, such as specifying meeting times or client visit schedules.

The date format is set according to the date format specified in the Jira settings.

Attachment enables users to upload multiple files to the form. It is commonly used for tasks like uploading quotes for expense approval or attaching supporting documents to an issue. Once uploaded, the documents are attached to the form as references or additional information.

URL is usually used for capturing simple responses. It is specifically designed for entering website addresses or links.

Numeric element is usually used for capturing simple responses. It is used for numeric inputs such as quantities, ages, or prices.

Content elements

Heading is used to emphasize and visually distinguish sections within a form. It serves as a prominent title or heading that helps organize and structure the form's content. By utilizing the Heading element, users can easily identify different sections and navigate through the form more efficiently. It acts as a visual cue that aids in providing a clear and intuitive user experience.

Content text box is designed to include additional information or create comprehensive and easy-to-understand specific instructions within a form. It provides a space for conveying details, requirements, or helpful tips to the form assignee, ensuring they have all the necessary information to accurately fill out the form.

This element allows for:

  • Text formatting options: Bold, Italic, Underline, Clear Formatting

  • Bullet list and Numbered list capabilities

  • Ability to insert Links, Emojis, Date, Divider, Expand, Info panel, Quote, Table, Status

Embedded content facilitates the direct integration of multimedia content, such as YouTube videos or images, into a form. It offers the convenience of uploading and displaying visual or audiovisual media within the form itself. By providing a designated area for adding a website embed URL, the Embedded content element enables the seamless inclusion of relevant external content, enhancing the form's engagement and versatility.

Horizontal Line serves as a visual separator between different form elements. It creates a distinct horizontal line that helps organize and structure the form's layout, enhancing its readability and visual appeal. By effectively demarcating sections or grouping related form elements, the horizontal line element contributes to a cleaner and more organized form design.

Response required indicates whether a user's input is mandatory or optional when filling out a form or providing information. When set to "yes," it means that the user must respond to the corresponding field to proceed. If set to "no," the user can choose to leave the field blank or skip it without any obligation to respond. It appears when a user clicks on .

Label is the descriptive text or question assigned to a specific input field within a form or user interface. It serves as a concise and clear identifier, informing users about the type of information they are expected to provide.

Description is additional information about the question mentioned in the label. It provides more context or clarification, helping users understand the specific details or instructions related to the input field.

Consent text is the predefining of text within a Consent checkbox.

Options provide a predefined list of choices or text options for a form element. By offering a set of predetermined options, form creators can standardize and limit the range of responses, ensuring data consistency and facilitating easier data analysis. There 2 types of abilities to fill out options for form creator:

  • Manual add allows you to fill options by hand with your own values.

  • Populate from the issue field allows you to fill options automatically with values stored in a specific Jira issue field.

Help text is an additional informational text provided alongside the form element. Its purpose is to offer further guidance or clarification to users regarding the expected input format, specific instructions, or any other relevant information.

Placeholder allows the user to set a temporary hint or example text within the form element. This text is displayed in a lighter color and disappears as soon as the user starts typing. The placeholder text provides a visual cue or prompt to the user, indicating the expected input or providing an example of the desired format.

Min/Max Characters allow the user to define the minimum and maximum number of characters that can be entered into the form element. These settings provide a way to enforce specific length constraints on the input. If the user tries to submit a value that exceeds the maximum or falls short of the minimum character count, an error message will be displayed.

Related URL allows the user to provide a web address (URL) that offers more information or instructions on how to submit the form element properly. This URL serves as a reference or resource for users who may require additional guidance. By including a Related URL, the user can ensure that individuals have access to detailed instructions, examples, or any other relevant information that can assist them in correctly filling out and submitting the form element.
